胸主動脈腔內治療所造成的主動脈塑型效果

主動脈剝離是嚴重且可能致命的主動脈疾病。它起因於主動脈內膜的破裂導致主動脈有一至多個破口,並將主動脈分離成兩個腔室。一個是原本就存在的真腔(true lumen),另一個是較薄的管壁所組成的假腔(false lumen)。A型或Debakey第一及二型主動脈剝離仍以傳統開放修補手術為標準治療。但胸主動脈腔內治療手術(thoracic endovascular aortic repair, TEVAR) 已成為複雜性B型或Debakey第三型主動脈剝離的首選方法。由於胸主動脈腔內治療手術並未把有問題的主動脈切除置換掉,所以術後的主動脈塑型就顯得更為重要。在本篇論文中,我們利用不同的參數去研究近

端及遠端的主動脈塑型。首先,我們使用中心線為基準的主動脈角度算法去評估主動脈弓的角度變化以及新一代主動脈覆膜支架對於主動脈彎曲的順應度。我們發現使用新一代的主動脈覆膜支架的順應度比較好,比較少喙型貼合不良(bird-beak configuration)的產生。主動脈弓的角度變化也比較小。其次,我們研究束帶技巧(banding technique)在複合式第一型主動脈弓修補手術中對升主動脈的塑型效果。我們發現使用束帶技巧可以維持住升主動脈的管徑大小,不太會有主動脈覆膜支架移位或是第一型滲漏(endoleak)產生。再者,我們利用電腦斷層血管攝影的亨氏單位(Hounsfield unit)去定義

假腔的流量密度(flow density)。再用此流量密度去評估盲端效應(blind sac effect)在呈現部分栓塞(partial thrombosis) 的假腔的影響。我們發現在術前若具有高流量密度的假腔,其在接受胸主動脈腔內治療術後呈現部分栓塞假腔時,在早期可能會有胸主動脈擴大的現象。如果術前病人的主動脈原本就有擴大的現象,那病人在術後必須接受密切觀察以防有主動脈破裂的危險產生。最後,我們利用真、假腔的體積去評估在急性(acute)期及亞急性(subacute)期採用主動脈覆膜支架合併裸支架置放術(the provisional extension to induce comple

te attachment technique, PETTICOAT)治療複雜性Debakey第三型主動脈剝離的主動脈塑型。我們發現在急性期接受治療會有比較高且快的機會產生遠端主動脈良性塑型。從我們這幾個研究可以得知主動脈塑型是胸主動脈腔內治療手術與主動脈交互作用後的動態變化。主動脈覆膜支架的設計、主動脈本身的性質、以及醫療技術的進步,都會影響主動脈塑型。此外,我們在這幾個研究所使用的參數也為未來的研究提供新的觀點。

研究背景:在醫學、生理及運動競技表現領域均已證實高強度間歇訓練能提升心血管調控功能。而當人體受到地心引力以外的加速度作用,諸多心血管調控現象隨之發生。當戰鬥飛行時,因離心產生重力變化,容易發生重力昏迷,是不可忽視的重要飛安議題。研究目的:探討四週高強度間歇訓練介入對體位改變的心血管系統調控表現之影響。研究方法:召募空軍軍官學校男性大學生為試驗對象(10名,22.4 ± 0.5歲、172.4 ± 4.2公分、68.2 ± 8.5公斤),採用單一樣本重複量測實驗設計。受試者進行為期4週、每週2次的高強度間歇訓練(全力衝刺10秒、間歇休息50秒、重複操作10組)。比較訓練前、後的直立傾斜試驗之5分

鐘仰臥安靜時的心率變異性(低頻範圍功率、高頻範圍功率、低高頻功率比值及總功率)及血壓、心率(仰臥安靜時與直立一分鐘、三分鐘、五分鐘、十分鐘、十五分鐘、二十分鐘)等各項測試數值之差異。研究結果:經過4週高強度間歇訓練介入:一、訓練後的收縮壓變化值在體位改變的1分鐘(增加4.5%)及3分鐘(增加5.2%)均達顯著提升(p
